Near Miss Report

Definition: A near miss, near hit or close call is an unplanned event that has the potential to cause, but does not actually result in human injury, environmental or equipment damage, or an interruption to normal operation.

Note: You need to have a Safety team setup or at least 1 person setup for the Safety Module to work correctly.

Purpose: To report these unplanned events.

Navigate to Safety > Near Miss Reports

This should take you to a blank form

Proceed to fill out the form.

If you click the calendar icon on Near Miss Date and Time, a calendar will come up where you can select the date. You can scroll by month, or even year.

Clicking clock icon will bring up a dropdown list of time (in 30 minute intervals) where you can log the approximate time of the Near Miss.

Pro-tip: if you want to log an event that happened to day at say 10:30am, just hit the clock icon and select that time, and the date (today) will automatically fill in.

The Division dropdown must be selected before the Location dropdown will populate with values.

The Attachment field will let you add attachments to the BBO

  1. will open the attachment in a separate browser window.
  2. will download the attachment.
  3. will delete the attachment.
  4. will open a dialog to upload a new attachment

You can rename the file.

Contacts can be selected from the dropdown.

(optional)  Visit to Contact Dropdown Setup page to add or remove contacts from the Safety Team role.

Once all the necessary fields have been filled out, hit the Submit button.

Wait half a second for the form to validate, then you will see a green success message.

You will be redirected back to the same page, with a new blank form (for use-cases where multiple forms need to be submitted)

If you wish to view the NMR you just created…

Navigate to Safety > NMR Supervisor Report (assuming you have permissions)

This will bring to a page with a grid of all the completed Near Miss Reports

Oh look, there is the report we just submitted.

The grid is filterable and sortable by field. By default, the grid is sorted  Near Miss Date & Time (most recent at the top). Clicking the blue arrow will reverse sort the grid.

There are also actions users can take on this page.

  1. will open NMR in Edit mode
  2. will open the audit dialog (show when NMR was created, modified, and by whom)
  3. a new NMR (takes you to a Create page)

If you click the pencil icon, you’ll be taken to your NMR in Edit mode.

Once the necessary changes have been made, hit Submit and you will be redirected back to the Supervisor page.
