Supervisor Incident Investigation (SII) Report

Definition: An incident investigation is the account and analysis of an incident based on information gathered by a thorough examination of all contributing factors and causes involved.

Purpose: To create reports of safety injuries / incidents that happened in the workplace.

Note: You need to have a Safety team setup or at least 1 person setup for the Safety Module to work correctly.

Navigate to Safety > SII Report

This will bring you to a page where you can see all the completed reports in a grid. To create a new report, click Add.

This should bring you to a blank form

The fields that require dates are capped at today’s date. I.e. you can’t future report an incident.

Clicking the clock icon on Incident Date & Time is expeditious way to get today’s date with a the time you selected.

Order is important. Division must have a selection before the Location dropdown will have values.

Hovering over the yellow dots on the body map will open a tool-tip where you can check body parts that were affected in the incident.

The Select files button will open a dialog and allow you to upload attachments to the report

The Other and Other – Explain checkboxes, when clicked, will open a new textbox where you can add custom text describing your custom PPE Worn or Incident Cause.

The Manager / Supervisor Signature will show up after the Manager / Supervisor Name Printed field has been entered.

These contacts can be set up via the Contact Dropdown Setup page, located in Configuration > CRM Setup. If not created, a new role with the Title: Safety and Description: Safety Team will need to be created, and users added to that role.

After hitting Submit, the contact(s) you selected will receive a notification email with basic information that a report was created.
