Create Safety Forms which an employee can complete and submit for review. Those active Safety Forms will be available within the "Safety" menu (Figure 1). Creating and managing Safety Forms is done from the "Configuration -> Safety Setup -> Safety Forms" screen (Figure 2).
Note: You need to have a Safety team setup or at least 1 person setup for the Safety Module to work correctly.

Add Safety Form
- Click on the [Add new form] (Figure 3 orange outline). This will create and open the new form with the preset default items included (Figure 4).
Edit Safety Form
- Clicking on the first button (Figure 3 green outline) to the left of the Form Name you wish to edit. This opens the Edit Safety Form page (Figure 4).
Delete Safety Form
- To Delete a Safety Form, simply click on the [X] button to left of the Form Name you wish to delete.
Note: Safety Forms with submitted items cannot be delete.
Duplicate a Safety Form
- Click on the third button (Figure 3 purple) to the left of the Form Name you wish to duplicate. This will make and open a copy of the Safety form.
Audit Information
- Clicking on the forth button (Figure 3) to the left of the Form Name, this opens a modal containing Created By, Date Created, Modified By and Date Modified information.

Edit Safety Form: Sections
- Safety Form Title, Menu Item Name Order BY and Active. (Figure 4 orange outline)
- Safety Form Title: Used for the page tile when completing a Safety Form (Figure 11).
- Menu Item Name: Used for the creation of the Safety Menu items (Figure 1)
- Note: The Supervisor Report Items use the Menu Item Name suffixed by " - Supervisor Report"
- Order By: Used to determine the order within the Safety Menu (Figure 1)
- Active checkbox: Determines whether the Safety Form is shown within the Safety Menu. (Figure 1)
- Note: Safety Forms cannot be made Active if the minimum Required Items are not found. (See Required Items for Safety Form activation)
- Note: If a Safety Forms with submitted items is disabled, the "Safety Form Name - Supervisor Report" menu item will remain (Figure 1).
- Safety Form Items (Figure 4 purple outline)
- All Safety Forms are created with five default form items. (Observer Name, Date of Observation, Division, Location, Area) see (Figure 5) These items cannot be removed or disabled.
- Observer Name: Text Input field
- Date of Observation: Date/Time selection field
- Division: Dropdown list comprised of the Safety Divisions found in the Configuration -> Safety Setup -> Safety Division records.
- Location: Dropdown list comprised of the Safety Locations found in the Configuration -> Safety Setup -> Safety Locations records.
- Area: Text Input field
- Safety Form items may have parent -> child -> sub item relationships. Allowing Safety Forms to have form field item groups and dependencies.
- Group example: Equipment type -> Forklift
- Dependent Items
- Parent items cannot be deleted if children or sub items exist.
- When the form is being filled out, dependent items are hidden until the parent choice is selected.
- Please refer to the Safety Form: Item Types section below for available form item options.
- All Safety Forms are created with five default form items. (Observer Name, Date of Observation, Division, Location, Area) see (Figure 5) These items cannot be removed or disabled.
- Safety Form Options (Figure 4 blue outline)
- Attachments checkbox: Gives the person filing out the Safety Form the ability to attachment files to the Safety Form.
- Comments checkbox : Gives the person filing out the Safety Form a text area to leave any observation's they have for the Safety Team.
- Notes checkbox and text area: When the Notes checkbox is checked on, text added to the Notes Rich text editor will be display at the top of the form. IE: instructions or notes about filling out the Safety Form.
- Safety Form Recipients (Figure 4 green outline)
- Assigned to list: This multi-select item is used to build a dropdown list of contacts the person filling out the Safety Form can choose as the primary recipient of the form.
- By default the Safety Team members are all preselected as possible Assigned to options.
- To the right of the multi-select control is the [+ Add] button. This button allows the addition of other company contacts not found as part of the Safety Team. (Figure 6)
- Forward to list:

- Assigned to list: This multi-select item is used to build a dropdown list of contacts the person filling out the Safety Form can choose as the primary recipient of the form.
Editing a Safety Form Item

- Editing any Safety form item is done by finding the item, then clicking on the Pen Icon (Figure 7 green) This will open the Edit modal box
- The editing modal box differs based on the type of form item type.

Safety Form: Item Types
- Checkbox: Used to select applicable items in a group of options.

- Ex. Submission Form view

- Date Picker: Used to enter a Date value.
- Date Time Picker: Used to enter a Date/Time value.
- Dropdown List: Used to select a single option from a list of options. This item type has a cascade function which is used to show/hide dependent form field items.

- If an option is used as a dependency the option cannot be deleted. (red outline)
- If an option is used as a dependency the Input Type cannot be changed.
- Radio List: Used to select a single option from a group of options.
- Text Area: Used for entering multi-line text.
- Text Input: Used to enter a single text value.
- Email Input: Used to enter an email address.
- Note: Cannot be used to include email recipients
- Numeric Input: Used to enter numeric values.
- Telephone Input: Used to enter telephone number values.
- Section Header: Defines a section header for the form.
- Image: Used to show an image above / below / between form field items.
- Line Item: Used to show a text line above / below / between form field items.
- The line item allows setting the text styles Bold, Italic and choosing between three text colors (Green, Red, Black)

- Pass / Fail / N/A Radio Group : Used to expedite the creation of Safety Forms
- Pass / Fail Radio Group: Used to expedite the creation of Safety Forms
- No / Yes: Used to expedite the creation of Safety Forms
Required Items for Safety Form activation
- A top level form item with a label containing the word "Type" (Figure 4)
- Click on the [+ Add Form Item] button seen in (Figure 4). This will open the Add New modal box. (Figure 8)
- Enter the form label. Ex. "MISC Equipment Type", "Type"
- Select the Input Type of Dropdown List. This will give you the Options section for adding the types of equipment.
- Click the [+ Add]
- Enter the NAME
- Click on the Update button to save the new dropdown list option.
- Continue adding any number of options needed. Minimum 1
- Once all the need options have been entered, click on the [Update] shown outlined green (Figure 8)
- A child form item with a label containing the words "Total" & "Assessment"
- Click on the arrow to the left of the "Equipment Type" (Figure 9 shown in orange)
- Click on the [+ Add Form Item] shown within the "Equipment Type" group (Figure 9 shown in green). This will open the Add New modal box (Figure 10)
- Enter the Form Item label. Ex. Total Conveyor Assessment
- Select the Dependent Value (Figure 10 shown in purple)
- Select one of the Pass / Fail radio groups for the Input Type.
- Click the [Update] button (Figure 7 shown in green)
- Some selection must be made to both the Assigned to list and the Forward to list.
Note: These two requirements are needed for the submitted Safety Form Supervisor Reports

- Enter the Menu Item Name (Figure 11). This text is used to assign the menu item name for the form (see Figure 1)
- Enter the Form Title (Figure 11). This text is used for the page title of the form. (Figure 12 orange)

Deleting a Safety Form Item
- Deleting a Safety form item is done by clicking on the [ X ] button (Figure 13) to the left of the item you need to remove.
- Note: Default and parent form items cannot be deleted.
